GLEN MILLS SCHOOLS’ PHILADELPHIA REINTEGRATION INITIATIVE

The Glen Mills Schools in conjunction with the Philadelphia Family Court and the Department of Human Service has launched a program of support and supervision which delinquent youth receive when they return to the community from placement. The Reintegration initial includes:

*A focus on reintegration needs from initial placement with on-going Reintegration Plan Determination of intensity of services based on the CANS assessment

*A team approach to reintegration with includes a Reintegration Worker (RW), Probation Officer (PO) and facility staff (counselor), family, and the youth.

*Family involvement and focus on community resources

*RW support throughout placement and for 3 months after discharge

*Focus on supporting the family’s welfare, school attendance, employment, and use of resources in the youth’s community.

Students are identified as standard or intensive based on the CANS assessment. For all youth the RW, PO and placement staff work together as a team to maintain a focus on reintegration needs.

This program is currently available for students of the Glen Mills Schools committed through the county of Philadelphia.
